Patients can pay their balance via the patient portal.

Medicare, Medicaid & Commercial Insurance:

Upstate Medical Associates, P .A., accepts assignment for multiple insurance plans, including Medicare, and will therefore file insurance claims on behalf of the patient. The patient is responsible for annual deductibles, co-payments and non-covered fees at time of services rendered. It is of importance to note that insurance coverage is a contract between the patient and their insurance company, and that UMA does not define coverage.

Accident Insurance:

UMA does not hold assignment on personal injuries. Therefore, the patient is required to pay required fees at the time of service. UMA does not file claims for auto and personal injury accidents, but per request, will provide documentation needed for patient to file claims.

Worker’s Compensation:

Worker’s Comp claims will be filed to the appropriate carrier per your loyer. If the claim is denied, the patient’s personal health insurance will be filed. Subsequent deductibles, co-pays are the patient responsibility and due at time of service.

Self Pay:

Self pay patients are required to pay charges rendered at time of service.

Charges, co-pays, co-insurances and deductible payments are required at time of service. Failure to complete payment may result in accounts sent to collections, or discharge from the practice.
